As a reminder, all participants with an appointment time must:

• Watch a brief orientation that explains the benefits and limitations of the card

• Provide proof of identification (passport, embassy ID, driver's license, or national ID card).

• Proof of current address (utility bill, bank statement, health record, lease agreement).
